When Neck Manipulation goes wrong!!! 😱. ————————————————— I have had a some DM’s from people asking if cervical manipulations are dangerous so I thought I would clarify some concerns on the issue.——————————————————— A recent systematic review by Nielsen, et al, 2017 concludes that the incidence of an adverse event involving a cerebrovascular accident is 1/400,000 manipulations. ———————————————————- Unfortunately, The case report in question revealed that this person was already walking into the clinic showing signs of red flags and contraindications to manipulation in the first place!! ———————————————————- The review suggests that cervical manipulations are safe and the risk of adverse events is low if done with sufficient judgement and examination by the clinician as well as adequate skill and appropriate use of techniques.——————————————————— A good clinician knows they are not always indicated but are completely safe if done at the right time with the right patient.

DO YOU WANT TO DELVE DEEPER INTO #b2rhealth METHODS AND LEARN HOW TO MAKE YOUR REHABILITATION EVIDENCE BASED, PERSON CENTRED AND IMPROVE ADHERENCE (FUN)?? In this example we are exposing Byron to some challenging proprioception tasks in his rehabilitation following ankle reconstruction following chronic instability. It has been shown that the Star Excursion Balance Test (SEBT) or ‘Y’ test can expose chronic ankle instability in both pre and post operative cases - and in particular posteromedial to stance leg (Plisky et al 2009). There are many uncertain variables around the setup of this test - but training the test can improve functional outcomes (Chaiwanichsiri 2005). We set this drill up with some tape on the floor and the different arms of the star enable me to provide some constraints (ie - stance foot can’t move, reach leg doesn’t touch the floor, strike ball with foot, don’t get hit by the ball....these change based on response) - then I can specifically spend time conditioning / exposing to those supposed corners of the star that need conditioning. Very fun, but very clinically relevant. El dolor que aparece en la zona cervical de la columna, sobre todo detrás del cuello, recibe el nombre de cervicalgia. Puede estar motivado por una rectificación de la cervical, estrés o una mala postura. También suele aparecer acompañado por otras molestias que pueden concretar aún más el verdadero origen del problema.⠀ ⠀ Una de las molestias más frecuentes hace referencia al dolor de cuello. Es una molestia que no tiene nada que ver con la tortícolis. Apreciarás una rigidez y tensión en esta zona del cuello, que con el tacto se intensifica y que se calmará cuando echemos el cuello para atrás. 🔥It All Starts With The Feet!🔥 _ ⚕️Improve performance and reduce injury risk by following @activeathletictraining⚕️ _ Great post by @amonferrymd ➡️ 👣 Here I provide the three main classifications of foot strikes as well as the optimal type of running shoe for each class:  _ 👣1. Overpronation (Pronation): Occurs when the arch of the foot flattens as the foot strikes the ground, causing the ankle and foot to roll inward. This places large amounts of stress on the ankles as well as the tibia, which may develop into ‘shin splints’. If you overpronate you’d benefit by choosing a stability shoe to help evenly distribute the impact. _ 👣2. Neutral: Occurs when the foot neither pronates nor supinates when striking the ground. This is the most efficient and safest manner to walk/run. If you have neutral foot strike, your foot rolls inward about 15 percent and optimally distributes the forces of impact, so you'd choose from a variety of shoes including a neutral-type. _ 👣3. Supination (Underpronation): Generally occurs in people with high arches. The foot strikes on the outer side of the foot causing an outward role of the ankle. Supinators are at a higher risk of developing ankle sprains, plantar fasciitis, and foot fractures. You’ll typically want to be in more cushioned or neutral shoes that allow your feet to pronate more. Many of the big brand names, like Nike, Asics, and Saucony have supinator-friendly shoes to get you going. _ Please let me know if you have any questions

ILIOTIBIAL BAND SYNDROME • Iliotibial band is a thickening of the fascia lata on the lateral aspect of the thigh.↘🏌 This structure runs from the iliac crest and attaches to the Gerdy’s tubercle of the tibia. • Iliotibial band syndrome (ITBS) is also referred as iliotibial band friction syndrome. This condition is a common cause of lateral knee pain especially among runners.🏃 The pain is thought to be initiated by friction between the ITB and the lateral structures of the knee.🔥 Contrary to common belief the iliotibial band is also attached to the femoral epicondyle which refutes the friction theory.🕸 • The treatment for iliotibial band syndrome should focus on decreasing strain (by modifying activities and resting), strengthening the hip abductors and increasing motor control.🚶 • There are multiple factors involved in the iliotibial band syndrome but further research is still needed on the condition. 📚 • HERE’S 2 EASY EXERCISES FOR ITBS ✔HIP ABDUCTION STRENGTH ✔HIP HIKE MOTOR CONTROL

How putting all your bodyweight on one leg can screw up your back? . I always emphasize to patients about knowing how to feel a connection with their feet to the ground (how do you ground yourself down like a sturdy) - as you can see, if you're the type of person who loves to just sag your pelvis off to one side and totally load up one leg like a crutch when standing, the long term issues can lead to all these different things happening upstream - all of a sudden 3 months later all you can complain about is this nagging back issue that seems to be an annoyance to resolve! . You keep away at the site of pain, forgetting to address and adjust and bring awareness to what the feet are doing. If your hip and your feet aren't communicating, they're not going to want to work in synchronization to create a nice stable base for your torso, chest, upper back, neck, shoulders and head to sit nicely on 🔝 of. Solution? Learn to stand still. If you don't want to, then learn to play hot potato (constantly move, don't let these dysfunctional patterns solidify)! . Disc bulge, disc prolapse, slipped disc, disc protrusion, herniated disc ... essentially it all means the same thing. That part of the disc material is compromised and is ‘bulging’ out. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ Discs sit in between the vertebra of the spine and act as cushions that absorb the shock when we use our bodies. They will start to wear as we get older, which is completely normal. In medical terminology it can be called ‘disc degeneration’ or ‘degenerative disc disease’ ... these are scary sounding words that mean WEAR AND TEAR! ⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ The amount of disc ‘abnormality’ is NOT directly proportional to the level of pain. Individuals can have the smallest disc herniation visible, but be in severe pain. On the other hand, disc herniations are common amongst people that have NO back pain! This highlights that MRIs are not pain scans and a thorough physical examination can be far more useful in the diagnoses of pain. ⠀⠀⠀⠀⠀⠀ ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ Due to this, MRIs are not ordered right away, as it’s been shown to actually worsen clinical outcomes in some cases. It can create fear, anxiety and allows the imagination to fixate on structures contributing to a poorer prognosis. ⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀⠀ It is well known that disc herniations can completely resolve/reabsorb with time and conservative care. This can take 3-9 months, dependant on the individual. So surgery is not always necessary and should be used as a last resort.
