I had a few questions about the knee meniscus and what is actually looks like after my post last week, so here’s a little look at the knee joint anatomy from behind. We have meniscus on both the inside (medial) and outside (lateral) section of the knee joint. They are basically rubbery disks of cartilage that act as shock absorbers and they cushion the joint cartilage and bony surfaces to reduce stress. The articular cartilage is the smooth layer of tissue that covers the bony surfaces of the joint. This is different to the meniscus. The bony and cartilage surfaces are both super important for long term health and function of the knee joint. A healthy meniscus will help to protect these structures long term.

Se estima que cada año se presentan cerca de 11 millones de casos nuevos a nivel mundial, de los cuales el 80% se dan en países en vía de desarrollo, entre ellos, México. 🇲🇽 . La fatiga  es el efecto colateral más común del tratamiento del cáncer, con prevalencias que van del 60% al 96% en pacientes sometidos a tratamiento activo; y puede persistir durante meses o incluso años después del tratamiento exitoso. 😪🤕 . Anteriormente se pensaba que el ejercicio físico podría tener efectos negativos sobre la progresión de la enfermedad en el paciente con cáncer.🤥🤒 . El ejercicio físico es una intervención no farmacológica, que tiene efectos no solo en el estado de salud físico, también repercute en la salud mental y la funcionalidad de los pacientes. A demás, puede impactar en todas las etapas de la carcinogénesis.😃🏊 . Velthuis et al. observaron que una intervención con entrenamiento de resistencia durante 18 semanas reducía de manera significativa los niveles de fatiga en pacientes con cáncer en fase de tratamiento activo.🔝 . Galanti et al. plantean que la realización de ejercicio físico aeróbico a dos niveles de intensidad para reducir la fatiga, esto es, a baja intensidad (40% de la frecuencia cardíaca máxima) y moderada intensidad (60% de la frecuencia cardíaca máxima).⚕️ . Los principales mecanismos biológicos de respuesta incluyen: ▶️Disminución de peso, obesidad y adiposidad central.🐷 ▶️Cambios en los niverles de estrógenos y testosterona y de los niveles de hormonas metabólicas (Insulina y glucosa). ITB Syndrome part 3 - Gait Re-training . The main gait change that I would consider making with a patient with ITBS would be to look at step width. We know that an increased angle of adduction as demonstrated on the picture (taken from a client) which may be due to a trendelenberg gait pattern or a crossover gait pattern can have an influence on development of ITBS in Runner’s • Changing gait can be tricky and takes time however it may be a way to improve symptoms and hopefully reduce recurrence rates. Meardon et al 2012 demonstrated that a modest change in step width could impact the strain on the ITB and suggested an increase could reduce pain • We are fortunate enough to have a line down the middle of the treadmill belt which can help, I’ve heard of people using tape to mimic this also • What cues or techniques do others use to improve step width? • I will be posting the second series of exercises in the next post, thank you for reading

ITB Syndrome - managing workload • As I mentioned in the previous post often ITB syndrome is multifactorial but commonly training error is a large factor. • Here are 3 nice images showing how spikes in training/workload can contribute to the development of running injuries such as ITBS. • Il often discuss the acute:chronic workload as per @gabbetttim with runners to help them understand how they can optimise their training but often once I have reduced their pain and improved strength and got them back doing some running I will enlist the help of @jamesphillipssc to help program their ongoing strength and conditioning and delve deeper into their goals and build a program that enables them to achieve these (if possible) whilst reducing injury risk. • Unfortunately at the start of the injury there is commonly a period where we have to stop running or drastically reduce to enable the tissue to settle down, monitor pain for 24 hours if still running and if unable to run without a flare then ceasing for a period of time and cross training is the best course of action. • The next post will cover gait retraining which may be worth looking at with runners with ITBS

ITB syndrome / pain • Runners have a high chance of developing lateral knee pain at the insertion of the iliotibial band distally. This is most commonly developed due to training error. Training error could include: -too much too soon -too little variability in the training -inadequate rest -adding a new stimulus e.g. sprints/track work/hills • The key first principle is load management and adjusting the stimulus that is driving the symptoms and this will be covered in the upcoming posts • The video demonstrations are of a few of the exercises I prescribe commonly but not exclusively for ITB pain at the start • 1.Foam rolling quad and TFL not the ITB 2.glut release with a ball 3.hip thrust with a band 4.crab walks 5.captain Morgan/ glut Med isometric holds • There are many other options and exercises which depend on the person but these can commonly be a good starting point • Give them a try and let me know what you think 🤔 • The next post will cover the key area of load management and acute/chronic workload • @running.physio @physio_charlie @physiogram @puresportsmed @theprehabguys
