Lower Back Pain From Squats: Why this Happens and How to Avoid it When performed properly, squatting is unlikely to result in injury. However, the spine is the most vulnerable of the joints during squatting and you may experience pain here. 🤕 Back pain with squats? 🤕 - Today, we are going to give you guys a few generic tips that WE think can help those suffering from back pain with squatting. - Let’s get to it: 1️⃣. Loop band around knees 🔥 will help to prevent collapsed/valgus knees & help maintain equal knee/hip alignment, further reducing a right or left excessive loading bias on the lower back. 2️⃣. Dowel between feet 🔥 will ensure central body alignment, allowing a 50/50 weight distribution between the left and right knees, hips and lower back. 3️⃣. Heels raised 🔥 can help maintain an upright spinal posture= decreased force going through the lumbar spine. 4️⃣. Retrospective Cohort Study‼️ . Aim: To determine whether those who participated in physiotherapy had better pain and functional outcomes over an 18-month follow-up period vs those that did not. . Method: 1️⃣70 patients met the inclusion criteria and a baseline assessment was taken prior to the study. Follow-ups were made at 3, 6, 12, and 18 months. 55 patients were included due to dropout 2️⃣Diagnosis of RC tear was made by a sports or shoulder fellowship trained attending physicians and evidence of structural deficit on MRI. 3️⃣All patients were given referrals for physical therapy and at 3 months patients responded whether they underwent physical treatment. . Outcome measures: Shoulder Pain and DIsability Index (SPADI) . Results: 1️⃣55 patients were used for analysis after drop out. 2️⃣18 patients responded they did not undergo physical therapy and 37 patients responded they went to physio within the first 3 months. 3️⃣Those who underwent physio → (86% performed strengthening exercises, 76% stretching exercises, 22% underwent modalities). . Patients who underwent physio within the first 3 months had significantly better shoulder pain and function compared to those who didn’t at 3 months follow-up. (Depending on which value used for the minimal clinically important difference). However, differences were not significant at later follow-up periods. . Limitations: Small cohort of 55 patients. Many variables were patient-reported and unverifiable. No standardized physical therapy protocol was used (types of exercises, duration of sessions, etc.) . 📚 Physical Therapy versus natural history in outcomes of rotator cuff tears: the Rotator Cuff Outcomes Workgroup (ROW) cohort study. Dickinson, et al. (2018).

Often we will finish off a treatment by using Kinesio tape on our clients. When the tape is applied correctly, it helps by lifting the skin to create a small space between the muscle and dermis layers. That space takes the pressure off swelling or injured muscles, allows smooth muscle movement and makes space for drainage and blood flow. Traditional athletic taping would restrict blood flow, however Kinesio tape is pliable to allow a full range of movement. 🔥💥 Return after knee injury. It would make sense that following a knee injury, there should be a stage between moment of injury & return to play, right? Of course. This is the rehab stage which if done properly should be periodised into cycles. It shouldn’t be random choices. It should be well planned with a clear objective. After finishing my last cycle, which had a large amount of high intensity work, I picked up a little niggle in my right knee (patella tendon). First reaction was to figure out why. If it went through the same load as the left, then why did the right tendon become painful? Secondly, after a very brief period of offloading, we began adding in isometric loading to the quads by using weighted wall sits. All has been going well so yesterday we included these kettlebell front rack squats, to a tempo of 2 seconds down and 2 seconds in the bottom. 5 sets of 12 reps. Low intensity, high volume with no aggravation. I’ll continue building here with the hope of getting back to where I was. 💥🔥

🔥💥 The 90/90 stretch. It’s definitely one of my favourites to perform. It feels good and is great at freeing up the hips. Whilst static stretching lacks conviction in the research, I’ve always found a direct link with holds of 2 minutes plus and moving better & being more mobile. If you’re going to try this stretch I want you to concentrate on deep and slow breathing. Be consistent & you will see improvement. 💥🔥

Yes Mrs. Jones, we’re not miracle workers. Truth is healing also takes time. Research and experience has allowed us to estimate a healing timeframe. However some patients may heal faster and some may heal slower depending on factors like compliance, age, weight etc. On average, bones take around 6-12 weeks to heal, muscle strain 4-6 weeks and tendons 8-10 weeks. I have a few docs that are adamant about it and some that are 💯 against it. . I asked my good friend @zeeadd from @igniteohyzio to look into the research on this topic. Here is an excerpt from his full post over at chrisbutlersportspt.com . WHAT does the literature actually say? That’s what I set out to find out when asking two questions: ❓Should the athlete be braced ealry on following ACL reconstruction? ❓Should athletes be braced as they near return to sport (RTS)? . 📚POST OP BRACING Lindstrom et al. found that when bracing was randomized in patients during the first three weeks post-ACLR, there were no differences in joint effusion at the 3- or 12-month marks after surgery. . In regards to knee stability, Smith et al. found that acute bracing is used in the initial week or two of recovery in order to protect the quadriceps inhibited knee from sudden flexion under weight bearing loads. . This can provide both an external support in addition to peace of mind to the athlete knowing that they are supported during a vulnerable period of the rehab process. . Although these are just two examples, we haven’t seen enough research in support or against the use of bracing in the early stages of the rehab process to conclusively lean in one direction or the other. . 📚RETURN TO SPORT BRACING When taking a look at functional bracing on athletes upon return to sport, we find evidence both in favor of bracing and against its use. . Smith et al. found that functional bracing does not protect the reconstructed ACL nor improve long-term patient outcomes when compared to non-braced participants. . ACL Rehab - Quads Focus —— Regaining strength is a lengthy task following ACL surgery. One of the primary groups of muscles affected after surgery are the quads. Significant research supports regaining at least 90% quad strength when compared to the non-surgical knee. I personally believe that many people don’t achieve this level of strain which contributes to the high ACL retreat rates. This can be due to the therapists lack of knowledge for progressive strengthening exercises or the facilities lack of equipment. This is a HUGE problem which is why I push for late stage ACL patients to get into a facility similar to ours. ——- Here are two quad focused exercises I like to use in rehab. 1️⃣ TRX Combo - this is a combo between a pistol and skater squat. The quad doesn’t get much time to rest between the two motions and this one is extremely taxing on the quads. 2️⃣ Skater Squat - I’m a big fan of the skater squat as it not only challenges the quads but also works on single leg balance. An exercise known to help make your back and hips STRONGER might actually also make them HEALTHIER! - 📚 Welch et al looked at the effects of a 16-week free-weight resistance training routine on patients with lower back pain for greater than 3 months in duration - 🏋🏻In addition to deadlifts, they included goblet squats, lunges, planks, and step ups - 💪🏼They also had individuals lifting loads between their 6 and 10 repetition maximum - 📈The results showed significant improvements in fatty infiltrate of the lumbar muscles, a 72% decrease in pain scoring, a 76% improvement in disability measurements, and increases in quality of life scores! - 📚Another study (Asa 2015) took people with greater than 3 months of low back pain and split them into two groups: - 📊One group did 8 weeks of deadlift training while the other group did low load motor control exercises designed to SPECIFICALLY TARGET the impairments of the individuals that were found on exam - 📈After the intervention, both groups had significant decreases in PAIN intensity, and increases in STRENGTH & muscle ENDURANCE measurements - 📉The only statically significant difference between groups was in their Patient Specific Functional Scale (a questionnaire that specifically measures the specific functional limitations of the specific individual), which favored the low load group (not surprising since the exercises were INDIVIDUALLY TAILORED to the specific person) - 🗝 So what are some of the key takeaways? - 📌Deadlifts are not inherently BAD for your back 📌In fact, deadlifts can actually HELP to decrease pain while increasing strength and resiliency in the hips and spinal musculature - 🚨Does this mean that EVERYONE with back pain should be deadlifting? NO. ⁉️Or that deadlifting can NEVER contribute to low back pain if programmed or performed incorrectly? NO. - 🏋🏻However, the available research gives us reason to believe that the deadlift can be used not only as a high quality performance exercise, but for rehab as well 👌🏼

{ITB rehab - ABDUCTOR STRENGTHENING} 🍑 ⠀⠀⠀⠀⠀⠀⠀⠀⠀ While stretching, foam rolling, cupping, dry needling, hot packs, etc. can help decrease some discomfort in the short term, they’re not going to kick ITB syndrome for good! ⠀⠀⠀⠀⠀⠀⠀⠀⠀ ITBS is often the result of overactive TFL & underactive glute medius & minimus (leading to increased hip internal rotation and adduction). FOAM ROLLING - by no means a replacement for stretching or warming up properly. If it's not working for you - that's okay. Don't foam roll a tight area till you are tender and bruised (i have seen people do this to themselves). If nothing changes it's not the right tool for you. Various sources will tell you different things. When to do it, how to do it, how long for. I'd usually recommend no longer than a minute on one muscle area. With a pressure you can bare and running the roller the entire length of the muscle. I'd also recommend foam rolling when you believe you feel most benefit. For me, it doesn't do anything pre-workout. Warming up i need to be dynamic, doing a bit of cardio partnered with joint mobility work. I find it most useful when i am suffering Delayed Onset Muscle Soreness. I find it really helps reduce the discomfort and it feels less painful to move normally. With anything don't always take somebody else's word for it! We're all different in our training styles, ranges of movement and response to training stresses.
